I will only use your personal data to provide the service you have requested and store it carefully.GDPR requires me to clearly inform you how I use, store and destroy your personal data. Before or at your first appointment, I collect personal data on your Client Consultation form. I use your name, address and date of birth to identify you. I use Jotform as a third-party service provider to collect and store information submitted through forms on our website.

When you complete and submit a form, the information you provide is securely transmitted to and stored by Jotform on our behalf. This may include personal data such as your name, email address, phone number, and any other information you choose to provide.

Jotform implements appropriate technical and organisational measures to protect your personal data, including encryption in transit (HTTPS) and secure data storage. Data may be stored on servers located in European Economic Area.

Jotform states complies with applicable data protection laws, including the UK GDPR and EU GDPR. Where required, appropriate safeguards such as Standard Contractual Clauses are in place for any international data transfers.

Jotform acts as a data processor on our behalf and only processes your personal data in accordance with our instructions.

When your email address is collected through our website, which is hosted on Squarespace, for purposes such as responding to enquiries, providing requested information, and (where applicable) sending marketing communications.

When you submit your email address via our website forms, your data is securely processed and stored by Squarespace on our behalf. Squarespace implements appropriate technical and organisational measures to protect personal data, including encryption in transit and secure data storage.

We only use your email address for the specific purpose for which it was provided. Where we rely on your consent to send marketing communications, you have the right to withdraw this consent at any time by using the unsubscribe link included in our emails or by contacting us directly.

Squarespace may store and process data on servers located outside the United Kingdom or European Economic Area. Where this occurs, appropriate safeguards are in place to ensure your personal data remains protected in accordance with applicable data protection laws.

We retain your email address only for as long as necessary to fulfil the purpose for which it was collected, or as required by law.

I may enter your name and email address into an online mailing tool which allows me to keep you up to date with news and offers relating to my services -  I’d love to keep you up to date but you can opt out of this at any time, just let me know. Mailchimp’s privacy statement is here.

Medical, health and lifestyle information are all used to ensure I give you a safe and appropriate treatment. Any hard copy client Information forms and treatment records are all kept in paper format in a cabinet. After 7 years of inactivity  forms & records will be shredded and electronic records deleted. If, during treatment, the client is under 18 or there is litigation involved with the injury, records may be kept indefinitely according to the guidelines of my professional body.Still awake?
